1. Complete all parts of section A and D for every incident, however small.
2. Decide if it is a minor or a serious incident.
3. Minor incidents are those which can be dealt with by basic, simple treatment, and require no further medical follow-up.
Serious incidents are:
- Injuries or sudden illnesses which require hospitalisation,
- Where a patient is taken to seek further medical advice, (eg. An on-site medical centre)
- Where the patient refuses either of the above, but is advised to seek medical care within 24 hours.
4. Complete the appropriate sections in as much detail as possible.
5. Sign and pass the top copy to the PATIENT
6. Pass the duplicate copy to the First Aid Administrator.
7. Where significant amounts of equipment are used, a claim for replacements may be made to the First Aid Administrator using the appropriate forms.
8. Where a patient refuses treatment or advice (such as to go to hospital) record this on the form, and ensure the patient signs that they have refused treatment/advice. |
